Important Note

TB Screen (QuantiFERON gold) requires special collection tubes and instructions. Outpatients must be collected at a trained Spectrum Health outpatient collection site. Specimen must be received at SHRL Chemistry within 14 hours of collection.

Specimen Requirements

It is recommended collection staff is trained on collection of TB Screen QuantiFERON® (QFT) prior to collecting the specimen. Incorrect collection could lead to aberrant results.

 

Specimen Type: Blood
Container/Tube: QFT® blood collection tubes.

Note: Maintain QFT® Gold tubes- three tubes per patient, 1 Nil (gray), 1 Antigen (red), 1 Mitogen (purple) tubes at room tmperature (ambient).

Volume: 1 mL per tube, fill to the black line on the tube

Collection Instructions:

  • Collect 1 mL blood by venipuncture into each QFT® blood collection tube.
  • Tubes fill slowly - hold tube on needle for 2 - 3 seconds after flow ceases. If blood level is not close to the black mark on the side of the tube label, obtain another sample.
  • Immediately after filling, shake tubes ten (10) times just firmly enough to ensure that the inner surface of the tube is coated in blood (to dissolve antigens on tube walls).
  • Over-energetic shaking may cause gel disruption and could lead to aberrant results.
  • Label tubes with patient name, date of birth, date and time of collection.

Clinical Information

A positive result should not be the sole or definitive basis for determining infection with M. tuberculosis. It should be followed by further medical evaluation for active tuberculosis disease (e.g., Acid fast bacilli (AFB) smear and culture, chest x-ray).

 

Diagnosing or excluding tuberculosis disease, and assessing the probability of Latent TB Infection (LTBI), requires a combination of epidemiological, historical, medical, and diagnostic findings that should be taken into account when interpreting QuantiFERON Gold IT results. Multiple Labels should print for each TB Screen order, it does not matter which suffix letter goes on what tube.

Regional Hospitals are offering the T-spot (Transplant TB Screen) test.

Transplant TB Screen (aka Tspot) is only for patients with immunosuppressive conditions or those undergoing treatment with immunosuppressive drugs.
