Important Note

Tier 1 Testing:  Hemoglobin (HgB) Fractionation is performed at Spectrum Health Regional Lab and should be orderd first when assessing if hemoglobin variants and Thalassemias are present.

Beginning March 4, 2020: A CBC w/o differential will be ordered on all orders for Hemoglobin Fractionation that do not already have a CBC ordered in the past 30 days.

Specimen Collection: Blood

Container(s): Full Lavender Top (EDTA) 3.0 mL 

Preferred Volume to Collect: 3.0 mL 

Minimum Volume to Collect: 1.0 mL

Neonate Volume to Collect: 1.0 mL

Capillary collect ok: Yes

Microtainer acceptable: Yes

Collection Instructions:

  • Gently invert lavender tube(s) 8 - 10 times after collection.
  • In cases where fractionation may be needed at a day or time when testing is not usually performed, the ordering provider must contact pathologist on call for approval.

Reflex Information

  • CBC (LAB294) on all orders for Hemoglobin Frationation that do not already have a CBC ordered in the past 30 days
  • If Hgb S is presumptively identified, a Sickle Cell Screen will be performed as a reflex confirmatory test.
  • If Hgb S is suspected in an infant less than 6 months of age, Capillarys Hemoglobin Electrophoresis will be performed as a reflex confirmatory test.
  • If Hgb C is suspected, Capillarys Hemoglobin Electrophoresis will be performed as a reflex confirmatory test.
  • If Hgb A2 result is >10%, Capillarys Hemoglobin Electrophoresis will be performed as a reflex test to confirm Hgb E.

Available Monday and Thursday, usual TAT 1- 4 days.
